Police are appealing for public assistance to help locate missing teenager Hayden.
The 15-year-old was last seen in Traralgon on 30 June.
He is described as 170cm tall with a slim build and brown hair and may be wearing grey tracksuit pants, a black t-shirt, a black Nike hoodie and white shoes.
Police have concerns for his welfare due to his medical conditions and lack of contact from him.
He is known to frequent the south eastern suburbs and Gippsland areas.
Police have also released an image of Hayden in the hope someone recognises him and can provide information regarding his current whereabouts.
Anyone with information about Hayden is asked to contact Traralgon Police Station on (03) 5172 2700.
