Police are appealing for public assistance to help locate teenager Jasmine Lumsden, Victoria Police say.
The 16-year-old was last seen on 11 May at her Wendouree home.
Police hold concerns for Jasmine’s welfare due to her age.
She does not have her phone with her.
Police have released an image of Jasmine in the hope someone recognises her and can provide information on her current whereabouts.
She last seen wearing a grey jacket and denim-look leggings.
Anyone who sights Jasmine or has information on her whereabouts is urged to contact Ballarat Police Station on 5336 6000.
