Police are appealing for public assistance to locate a 26-year-old man reported missing from Morayfield, 1 August.
Jacob Smith, pictured, was last seen at an address on Graham Road around 10.48am on 30 July.
Family and police have concerns for the man's welfare due to medical conditions, further Jacob has had no contact with family or friends, which is out of character.
Jacob is described as approximately 187cm tall, blue eyes, slim build with dark brown curly hair with a beard.
He was last seen wearing seen wearing black tracksuit pants, white & black adidas sneakers and a blue & grey checked flannel jacket.
Police are appealing to for Jacob, or anyone with information regarding his whereabouts, to come forward.
