Police are appealing for public assistance to find missing Kew man David Wheeler.
The 65-year-old was last seen leaving his Aged Care Facility in Cotham Road, Kew about 4pm on Thursday 3 September.
He was wearing a black jumper and grey tracksuit pants.
Police and David's family have concerns for his welfare as he suffers from dementia and has no phone or access to money.
He may be travelling on the public transport network.
Investigators have released a photo of David in the hope someone recognises him.
Anyone who has seen David or has information on his whereabouts should call Boroondara Police Station on 8851 1111.
