Police are appealing for public assistance to help locate missing Ringwood teenager Grace Van Der Ploeg.
The 14-year-old was last seen at a residence on Ireland Street, Ringwood about 9pm on 10 November.
Police and family have concerns for Grace's welfare due to her age and a medical condition.
Grace is Caucasian, approximately 170cm tall with a thin build and long blonde hair.
She was last seen wearing a white singlet top, blue denim skirt and converse shoes.
Investigators have released an image of Grace in the hope someone may have seen her and can provide information regarding her whereabouts.
