Police are appealing for public assistance to help locate missing teenager Jordonn Vitiello.
The 16-year-old was last seen in Flemington Road, Parkville at 5pm on 27 August.
Jordonn was last seen wearing a black Fila pullover, red tee shirt, navy tracksuit pants with grey/white stripes on the side and black runners.
He is described as 160cm tall, slim build with short shaved hair with a line on the left side, a rat's tail and has both ears pierced.
Police and family have concerns for Jordonn's welfare due to a medical condition.
He is known to frequent the Altona area.
Police have released an image of Jordonn in the hope someone knows of his current whereabouts.
Anyone who sees Jordonn or has information on her whereabouts is urged to contact Melbourne North Police Station on 8379 0800.
