New 10-year Framework And Program For Chronic Condition Launched

Our new national framework sets a clear national direction across 5 focus areas:

  • promoting health and education
  • preventing illness and detecting it early
  • providing joined-up, multidisciplinary, person-centre care
  • managing multiple chronic conditions better
  • improving support for priority populations to reduce health inequities.

A key element of our vision for health system reform, the new framework moves away from treating diseases in isolation and instead focuses on related chronic conditions and shared risk factors.

To support these priorities, our Chronic Conditions Prevention and Integrated Care Program will invest $109.9 million over 3 years from 2026-27, with ongoing funding of $38.3 million per year from 2029-30. We will provide funding through open, competitive grants to support:

  • innovation
  • collaboration
  • measurable outcomes.
