The 48-year-old worker was attempting to clear a jam from an industrial plastic granulator when his right index finger made contact with the machine and was severed.
A & J Australia Pty Ltd faces two charges under section 21(1) of the Occupational Health and Safety Act for failing to provide or maintain a safe working environment.
It is alleged the company breached section 21(2)(a) of the OHS Act by failing to provide or maintain plant that was safe and without risks to health.
WorkSafe also alleges the company breached regulation 97 of the Occupational Health and Safety Regulations by failing to identify hazards associated with the installation, commissioning and use of plant at the workplace.
The matter is listed for a committal mention at Melbourne Magistrates' Court on 6 January 2026.
