Police are appealing for public assistance as they search for missing man Peter.
The 77-year-old, who has dementia, left a hospital on Hastings Road in Frankston about 12.45pm and was last seen at Frankston Railway Station about 3.30pm on 13 April.
Police and family have concerns for Peter's welfare due to his age and a medical condition.
Investigators have released an image of Peter in the hope that someone recognises him and has information on his whereabouts.
Peter is Caucasian, of medium build with short grey hair.
He was last seen wearing a red and white Arsenal football shirt, maroon pants and shoes.
