Police are appealing for public assistance to help locate missing Crewsick girl, Jasmine.
The 16-year-old was last seen in Creswick on 24 November about 6pm.
Jasmine is Caucasian, of slim build, about 175cm tall and was last seen wearing all black clothing.
Police and family have concerns for Jasmine due to her age and medical conditions.
Police have released an image of Jasmine in the hope someone recognises her and can provide information regarding her current whereabouts.
Anyone who sights Jasmine or anyone with information is urged to contact Bacchus Marsh Police Station on (03) 5366 4500.
