Police are appealing for public assistance to help locate missing Colac man Barry.
The 59-year-old was last seen leaving Geelong hospital on 5 September about 2.30pm.
Police and family have concerns for his welfare due to medical conditions including dementia.
Barry is described as 175cm tall with a thin build with blue eyes, brown hair and facial hair.
Police have released an image of Barry in the hope that someone can provide information on his current whereabouts.
Anyone who sights Barry or has information on his whereabouts is urged to contact Geelong Police Station on 5225 3100.
