Christina Polomoff Running for ASCP Director-at-Large

Christina Polomoff, Pharm.D., a Population Health Clinical Pharmacist at Hartford HealthCare Integrated Care Partners, and an Assistant Clinical Professor at UConn School of Pharmacy is running for Director-at-Large for the American Society of Consultant Pharmacists (ASCP) Board of Directors. The election runs now through April 20.

Christina Polomoff, Pharm.D., a Population Health Clinical Pharmacist at Hartford HealthCare Integrated Care Partners, and an Assistant Clinical Professor at the UConn School of Pharmacy is running for director-at-large for the American Society of Consultant Pharmacists (ASCP) Board of Directors. Polomoff has served ASCP in a variety of ways, both locally and nationally. Currently she is on the board for the Connecticut ASCP chapter, and will be serving as interim-president for Connecticut this year. Nationally, she has co-chaired the National Medication Safety in Transitions of Care Committee for the past two years. In that capacity, she has collaborated with pharmacists across the country on ASCP's national priorities focused on raising awareness and creating/collating tools and resources focused on transitions of care.

At Hartford HealthCare Integrated Care Partners, Polomoff works with providers and care managers to ensure their older adult population receives the highest quality care at the lowest costs possible. Her background in population health, ambulatory care, and geriatrics showcases the unique roles that ASCP members can fill. As director-at-large, she would like to be a voice for these non-traditional practice settings.

Polomoff also serves on the board of directors for the Connecticut Pharmacist Association, and holds an appointment on the State of Connecticut Department of Development Services Institutional Review Board. She graduated from UConn with her Doctor of Pharmacy and Business Minor. She completed a PGY1 residency at the VA Connecticut Healthcare System followed by a PGY2 ambulatory care residency at The Johns Hopkins Hospital. She holds dual board certifications in ambulatory care and geriatrics.

Polomoff is excited about the opportunity to expand her service to ASCP members across the country.
