Wyndham City’s Community Connections and Care service has launched and is proving a valuable resource for residents needing free and confidential information, advice and support.
Community Connector staff are available to assist all residents in our community to access the services they need and empower them to make informed decisions.
Healthy City portfolio holder, Mayor Cr Adele Hegedich, said: “Wyndham’s Community Connector team can help residents in so many ways, over the phone, by email, in person – home visits in a Covid- safe way of course, to provide care and referral services to link our residents with the services they need.”
“The Community Connector teams support, inform and connect as well as tailoring specific information to meet the individual needs of residents and linking residents to a range of programs and services.”

“The range of services the Community Connector team provide are varied and depend on the needs of the person they are assisting. It can range from food relief and material aid to emotional and social support, counselling, linking with community groups and activities to information and assistance accessing services such as My Aged Care and NDIS.”
This new service is already providing support to 25 residents per week.
