Police are appealing for public assistance to help locate missing teenager Hayden.
The 16-year-old was last seen on Ascot Vale Road in Ascot Vale about 5.30pm.
He is described as 170cm tall with a slim build and brown hair.
He was last seen wearing all black clothing and a Nike hat.
Police have concerns for his welfare due to his medical conditions.
He is known to use public transport and may be in the Dandenong, Pakenham and Sale areas.
Police have released an image of Hayden in the hope someone recognises him and can provide information regarding his current whereabouts.
Anyone with information about Hayden is asked to contact Flemington Police Station on 9371 6100.
