A critical incident investigation is underway following a fatal crash in the state's Hunter region.
About 10.45am today (Friday 3 October 2025), police attempted to stop a vehicle on Buchanan Road, Buchanan.
The vehicle is believed to have briefly stopped, before leaving the scene.
A short time later, the vehicle crashed into a telegraph pole on the same road and caught alight.
The female driver and sole occupant died at the scene.
While she is yet to be formally identified, she is believed to be aged in her 30s.
A crime scene was established and a critical incident team from Newcastle City Police District will investigate the circumstances surrounding this incident.
The investigation will be subject of an independent review by the Professional Standards Command and oversight by the Law Enforcement Conduct Commission.
A report will be prepared for the coroner.
