Police are appealing for public assistance to help locate missing man Troy Perna.
The 39-year-old was last seen on Como Street in Alphington about 4pm on 20 April.
Troy is described as being about 175cm tall with a thin build, long brown hair and a long beard.
He was last seen wearing a white t-shirt and black pants and is known to frequent the Altona Meadows, Frankston and Geelong areas.
Police hold concerns for his welfare as he has a medical condition which requires medication.
Police have released an image of Troy in the hope someone recognises him and can provide information on his current whereabouts.
Anyone with information about Troy is urged to contact Collingwood Police Station on 8413 1700.
