My Aged Care Support Plans Now Available In My Health Record

From 9 December 2024, older people will be asked during their aged care assessment if they authorise sharing their support plan with My Health Record.

This integration of My Aged Care to My Health Record is the foundational connection between aged care and health systems.

For older people

Your support plan records what you discussed during your assessment and sets out the care and services that will best help you.

Having your support plan in My Health Record will allow health professionals, such as your doctor, to see your support plan. This will give them a better understanding of your aged care needs and help them give you better care.

My Health Record is a safe and secure place to keep your health information online.

You can provide or withdraw authorisation to share your support plan with My Health Record at any time.
