Police are appealing for public assistance to help locate missing infants Daniel and Illham.
The two were last seen on 27 October in Port Melbourne.
Police have concerns for the children's welfare due to health concerns requiring immediate treatment.
The children may be in the company of a family member but their whereabouts remain unknown.
Police have released images of the children in the hope someone may have seen them.
Anyone who sights them is urged to contact St Kilda Police Station on 9536 2666.
