Police are appealing for public assistance to help locate missing man Jake.
The 29-year-old was last seen at a medical facility on Arnold St, Box Hill at 3am on 21 March.
He is described as 175cm tall, medium build, brown eyes, black wavy hair and unshaven.
Jake is known to frequent the Wantirna, Ringwood, Box Hill and Dandenong areas.
Police have concerns for his welfare due to medical conditions and the length of time he has been missing.
Jake is likely to be wearing a green zipper jacket and walks with a distinctive gait.
An image of Jake has been released in the hope someone will come forward with information about his current whereabouts.
Anyone with information on his whereabouts is urged to contact Knox Police Station on 9881 7000.
