Police are appealing for public assistance to help locate missing woman Jade.
The 37-year-old was last seen in Frankston on 12 November about 4pm.
Jade is of average height with a slim build and brown hair.
She was last seen wearing a purple singlet top, grey pants and a black and white cardigan.
Police and family are concerned for Jade due to a medical condition.
She is believed to frequent Frankston, Frankston North, Mornington, Phillip Island and Sam Remo areas.
Police have released an image of Jade in the hope someone recognises her.
Anyone with information about her whereabouts is urged to contact Frankston police station on 9784 5555.
