Updates To Provider Management Plan

We have updated the Provider Management Plan (PMP/Form 32). This form must be completed by allied health providers to outline their treatment plan and support for an injured worker.

The PMP has stayed mostly the same since 2013. Based on provider feedback, we have made some small updates to make the form easier to read, complete, and submit.

What's new?

  • Clearer layout and design

    Updated colours and spacing make the form easier to follow.

  • Editable PDF with improved fields

    The form remains fully editable but now offers improved fields and tooltips to reduce errors.

  • Added requirement to put in email address

    Makes it simpler for us to contact providers when we need more information.

  • More space for treatment details

    Providers now have more room to outline treatment plans, frequency, timeframes, and any additional comments.

  • Added link to submit via Provider Connect

    This is the preferred method to submit the form to WorkCover Queensland.

  • Fax option removed

    Supports faster and more modern online lodgement.

When can the new PMP be used?

The new PMP is available now and can be downloaded from our website. For guidance on how to complete a PMP, please visit our webpage.

Providers can continue using the older version of the form until 30 June 2026.
