This week our Community Services team mailed out advice of new care arrangements commencing 1 March 2024 across Central Goldfields Shire.
The providers for the new, Commonwealth Home Support Program (CHSP) for those over 65 years are:
- Maryborough District Health Service
- Uniting AgeWell
- mecwacare
Intereach has been named as the new provider for the Home and Community Care Program for Younger People (HACC PYP).
Council staff will continue to provide services – and be available to assist in the transition until services finish on 29 February.
To ensure a seamless continuation of care, we ask all clients to respond to their advisory letters promptly. Please ensure you sign the consent for the transfer of your personal details to the new providers, by:
- mail (using pre-paid return envelope supplied with letter)
- in person by visiting our Customer Service Centre at 22 Nolan Street, Maryborough
