More Support For Dementia Sufferers Transitioning To Aged Care

Older Western Australians living with dementia will have more support when transitioning from hospital to aged care.

The Hospital to Aged Care Dementia Support Program (HACDSP) is set to launch at Bentley Health Service and will feature specially trained staff to enhance quality of care.

The program continues for up to 3 months post-discharge, to ensure continuity of care and includes support to aged care services.

HACDSP also provides training and clinical guidance to hospital staff, helping them deliver dementia-informed care during a person's hospital stay.

Delivered by Dementia Support Australia and funded by the Commonwealth Government, the program aims to improve discharge time and provide better support to people with dementia entering residential aged care.

Bentley Health Service has been selected as the first WA site, with work underway to establish arrangements and ensure the program can commence as soon as possible.
