
The Commonwealth Home Support Programme (CHSP) is an Australian Government Department of Health program that helps older people stay independent in their own homes and communities for longer.
The CHSP's main goal is to increase participant independence by focusing on building participant strengths and existing capacity, using their goals and motivations to inform every part of the assessment, planning and delivery of their tailored support.
Another major focus of the CHSP is to reduce in-home risks that may jeopardise a participant's ability to continue living independently if unaddressed.
It's all about working with and alongside older residents in our region, to help them continue living healthy, happy, and independent lives in their own homes.
How to get support
- Call My Aged Care on 1800 200 422 or register online on the Australian Government's My Aged Care website
- You'll be contacted to organise the required applicant assessment appointments
- The assessment process will include a visit to your home that can take up to an hour
- You are encouraged to have a support person or advocate with you during the assessment
- If you're eligible, the final step is a home visit from Council for a Work Health and Safety property assessment and to establish your personalised care plan
- Your needs and required support services are reviewed once a year

Eligibility requirements
- Over 65 years of age, or over 50 years of age for Aboriginal and Torres Strait Islander persons
- Still living at home
- Needing help to continue living independently
