Chronic Conditions Prevention, Care Program Launch

We have published the forecast opportunity for the upcoming Chronic Conditions Prevention and Integrated Care Program grants round on GrantConnect. This grant opportunity aims to improve health outcomes for people in Australia living with, or at risk of, chronic conditions by supporting early detection and effective management. Guided by the National Strategic Framework for Chronic Conditions 2026-2035, the program will invest strategically and competitively to ensure transparency, fairness and alignment with government priorities.

The objectives are aligned to the framework. These are:

  • health promotion and education
  • embedding prevention and early detection
  • system-integrated, multidisciplinary, person-centred care
  • managing multimorbidity
  • enhanced and targeted health equity support.

To assist prospective applicants, we have also published draft frequently asked questions (FAQs). These FAQs provide guidance on eligibility requirements, application procedures, assessment criteria, and recommendations for preparing proposals. We advise applicants to review these materials thoroughly as they contain relevant information regarding department expectations and approaches to optimising application success.

We will provide further information, including instructions on how to register for an upcoming public webinar. The webinar will give applicants an opportunity to engage with department representatives, ask questions, and learn more about the Chronic Conditions Prevention and Integrated Care Program.
