Police are appealing for public assistance to help locate missing Sebastopol man Simon.
The 45-year-old last spoke to family on 1 October but has not been heard from since.
Family and police are concerned as Simon has a medical condition that requires regular medication.
His car was located on Italian Gully Road, Scarsdale on 4 October and police have searched the area but are yet to locate Simon.
Police have released an image of Simon in the hope that someone may be able to provide information on his whereabouts.
Anyone with information on his whereabouts is urged to contact Ballarat Police Station on 5336 6026.
