DCV marine incident reports and safety lessons are now produced quarterly. Numbers may appear higher under this new format.
Between July and September 2025, we received 263 marine incident reports involving domestic commercial vessels. Of these, 41 were serious and 1 very serious.
Below are some of the occurrences included in this period.
- A contractor fell between a vessel and the jetty while disembarking. They were wearing a lifejacket and were recovered without injury.
- Three crew members exhibited symptoms of dizziness, light-headedness and disorientation after their shift. The crew tested positive for CO and CO2, with low oxygen saturation.
- An employee sustained a significant hand injury while attempting to release an aft rope from a bollard.
- Fuel spilled into a lit gas oven while refuelling a generator. Two vessels sustained significant fire damage. One crew member sustained serious burns and required medevac for treatment.
