Police are appealing for public assistance to help locate missing Warragul man Jake.
The 33-year-old was last seen in Warragul about 12.15pm on Thursday 5 June and last contact with Jake was on Monday 9 June.
Jake is described as Caucasian appearance, a bald head, dark brown beard, 182cms tall and a medium build.
It is unknown what Jake was wearing when he disappeared.
It is likely Jake is utilising a pushbike or public transport.
Police and Jake's family have serious concerns for his welfare as he has a medical condition that requires daily medication and his disappearance being out of character.
An image of Jake has been released in the hope it will assist in locating him.
