About the Study: Are novel autoantibodies associated with idiopathic myelopathy? In this case-control study, autoantibodies targeting the transcobalamin receptor (CD320) responsible for cellular transport of vitamin B12 were identified in a portion of individuals with idiopathic myelopathy, especially those whose disease mimicked subacute combined degeneration. Screening for anti-CD320 followed by metabolic confirmation of a central nervous system-restricted vitamin B12 deficiency may be considered in the diagnostic evaluation of myelopathy.
