An investigation is underway after a fatal muti-vehicle crash in Sydney's south-west this morning.
About 11.40am today (Tuesday 12 May 2026), emergency services were called to Canterbury Road, Lakemba, following reports of a two-vehicle crash.
Officers attached to Campsie Police Area Command attended and were told a sedan and an SUV collided nose to tail, suspending the SUV in the air and trapping the occupants.
The sedan continued travelling a few meters before colliding head-on with a Ute, trapping all occupants.
As a result, a further seven vehicles collided.
The driver of the sedan – a man believed to be in his 80s – died at the scene and is yet to be formally identified.
A passenger of the sedan – a 78-year-old woman – was freed from the vehicle by Fire and Rescue NSW and was taken to St George Hospital, where she remains in a critical condition.
Nine other drivers and their passengers were freed and assessed by NSW Ambulance paramedics for minor injuries. The drivers were taken to hospital for mandatory testing.
A crime scene has been established, and investigations have commenced into the circumstances of the crash.
A report will be prepared for the information of the Coroner.
