Police are appealing for public assistance to help locate missing 15-year-old Tiara Jayde Kocis.
Tiara was last seen at 10pm on Friday 5 June in Clifton Springs.
Police have concerns for her welfare due to her age.
Tiara has a thin build, fair skin and brown hair.
Police have also released an image of Tiara in the hope someone recognises her and can provide information regarding her current whereabouts.
Anyone with information about Tiara is asked to contact Bellarine Police Station on 5256 2698.
