Prahran police are appealing for public assistance to help locate Mangui Chman.
The 45-year-old man was last seen at a hospital on Commercial Road at about 4.30pm.
Police have concerns for Mangui due to a medical condition requiring urgent treatment.
It is believed he may be in the Prahran, St Kilda or Melbourne CBD areas.
Mangui is described as being of African appearance, 180cm tall and of medium build.
He was last seen wearing a blue puffer vest and a red and white chequered shirt.
Police have released an image of Mangui in the hope someone recognises him.
